Surgery report Procedure - Fracture repair - ulna Performed by - des Surgery time - 29 minutes Had a very hard time getting him deep and controlled. Sterile field was blown almost immediately. No major breaks but it was far from ideal. It took sime time to dissect out the proximal end as it was severely overlapped and surprisingly contracted. Mostly a simple transverse. Drove IM pin from prox ulna to fx, removed distal trocar tip, aligned the ends and drove distally. Near perfect alignment. Flushed and closed skin with 4-0 vicryl. Placed a cross pin prox and distal to the fracture. Applied BNP to pins and suture line. Covered suture line with tegaderm. Set proper curvature of wing, bent pins and created a sidebar with VTP. Placed fig-8 wrap. Recovery was quick.

Surgery report Procedure - Fracture repair - radius Performed by - des Surgery time - 25 minutes Dorsal approach. Had to make a pretty long incision to expose both radius fractures. Found them all easily. The distal end was MUCH longer than it appeared on the rad but it did take some careful dissection around the tendons of the extensor carpi radialis and supinator muscles to figure out exactly what was going on. Placed IM pin thru the intermediate piece then exited distally thru the distal piece. Removed the prox trocar tip, aligned the ends and drove into the proximal end. Flushed and closed the skin with 4-0 vicryl. Was tempted to repair the ulna today as well but decided to get a rad to see how well we did on the radius. Applied BNP to pin exit and suture line.Note: the pin exit was basically at the suture line. Bent the pin over and kept long in case we follow up with an IM-ESF tie-in in the ulna. Placed a fig-8 wrap and got post-op rad. The rad showed that we nailed the placement. The ulna is very misaligned but would probably heal side-to-side. Will probably repair the ulna on Friday.
