Surgery report
Procedure - Fracture repair - tibiotarsus
Performed by - des
Surgery time - 25 minutes

Bruising is green/resolving so probably 5-6 days old. Fx is rubbery
but bone ends are not freely moveable.
Lateral approach over the point where I could feel the end of
the proximal piece best. Bluntly dissected to exposed both bone
ends. Took a huge amount of manupulation and levering to oppose
the ends. The proximal fragment was much more solid than it appeared
on the rad. Drove IM pin from fx and exited at the stifle. Removed
the distal trocar tip, levered the ends back into opposition
and drove distally. Flushed and closed the wound in 2 layers
with 4-0 vicryl. Drove large cross pin distal the fx and one
very thin one in the proximal fragment about 1 cm distal the
tibial plateau. The rad shows multiple fissues so I debated whether
to drive one there at all. The thin pin drove very easily so
I was worried it wasn't seated well but it felt solid when done.
Set the rotation, bent the pins and created sidebars with VTP.
ROM at stifle and hock is great. Applied BNP to pins and suture
line. Covered suture line with tega. Covered fixation with vetwrap
and placed foam shoe. Recovery was quick. Bird was a bit bradycardic
and RR was a bit slow but he was mostly stable throughout.

Splint was very loose and slipped down and metal was exposed.
Replaced splint. Fx definitely felt bendable under the splint.
Got full ROM at right wrist.

------------- PHYSICAL EXAM FINDINGS ----------------
Head
Behavior and Neuro: BAR
Other: laying down
Body
Vent: soiled with mutes, tone not great
Wings
Right wing: 80% extension
Right ulna and radius: old, distal u/r fx
Legs
Right leg: a bit limp, was able to kick
Right tibiotarsus: closed fx

Laying down, BAR. Not kicking much with right leg, no deep pain response. Palpated a closed TBT fx. Right wing does not have full extension, healed distal u/r fx. 0.24 cc torb IM.

Anesthesized with iso via mask at 2.5% for rad. Rad confirms healed u/r fx and a midshaft TBT fx. Consulted des, will try to surgically repair. Applied cotton wrap, vet wrap, and splint + foam shoe to right leg. Did not wrap the wing. Cleaned mutes from vent.

Hoping after a few PT sessions bird will regain full ROM.
