Removed foot bandage. Similar to previous week. Small wound in center of pad. Looks good. Did not re-wrap, covered with newskin. Supra-orbital ridge had 3 mm scratch over right eye.

Looked in eyes for practice, did not see same odd feathering of pecten as in R4 RTHA

BAR. NOLO.

Anesthetized with iso via mask. Rad shows significant bridging callus already. The fixation is solid and pins mostly clean but there is a good bit of swelling along the lateral aspect between the cross pins. There does NOT appear to be any boney involvement.
No evidence of coracoid fracture that was previously reported. I think that was incorrect.

There is a mild, superficial bumblefoot wound on left foot.
Cleaned wound and pins. Dressed foot with Vit-E + telfa + foam shoe.
Covered fixation with vetwrap.

Pulled blood for CBC - pending

Surgery report
Procedure - Fracture repair - tibiotarsus
Performed by - des
Surgery time - 25 minutes

Anesthetic nightmare!
Very hard to knock down for rad and maintained a severe tachycardia
( > 300 BPM) throughout most of procedure. Was breathing very
shallowly so we had to bag the entire time. Went completely apneic
half-way thru (Dopram 0.55 ml IM). Then became arrhythmic and
bradycardic near the end (atropine 0.66 IM worked well). Needless
to say, we were rushing the entire procedure and did not chart
anything. Recovery was uneventful and he was actually quite stable
for the last 5 minutes.
Initially tried normograde placement of the IM pin but could
not find the distal end. Made small incision on medial side of
fracture. Found bone ends. Severe scarring and mal-alignment
so normograde would never have worked. The IM pin was still in
the proximal fragment entering thru tibial plateau so aligned
ends and continued to drive distally. Actually took a few tries
to get it aligned. Was not able to remove distal trocar tip so
measured the length as I drove. Flushed wound and closes fascia/skin
separately with 4-0 vicryl. Bird was doing really poorly by now
so very quickly placed cross pins. They were at very odd angles
so had to bend them in multiple planes to create sidebars from
VTP. Overall stability, rotation and ROM very good. Applied
BNP to suture line and pins. Covered suture line with tega. Placed
foam shoe and covered fixation with vetwrap.

2018-09-08 17:27 jmw

Went to condo to check for knuckling. Currently standing flat footed, though not weight bearing. Had just eaten so did not place shoe. Will check in AM to see if shoe is needed.
